Employees and contractors may now submit their forms electronically through the Paymaster website. All forms are secure and will be verified with the contact on the payroll account. Please have your employer’s email address available so that they may receive a copy of your forms for their files.
Direct deposit forms will be verbally verified with the payroll contact and must be supported by sufficient documentation (voided check, bank letter or deposit slip with employee/contractors name matching). Please have those available for upload before completing the onboarding forms.
To ensure accuracy, please open the PDF versions of the forms below for instructions and additional information for each entry before submitting your forms. Neither your employer nor Paymaster are responsible for errors due to inaccurate information being provided.

W-4 Form and Important Information
- FAQs 2022 W-4 Form – For all questions, please refer to the official irs.gov website for all specific questions regarding the form and instructions.
- Application to calculate and print 2022 W-4 Form – Use your best estimates for the year ahead to determine how to complete Form W-4 so you don’t have too much or too little federal income tax withheld.
- I-9 Form – Submit your I-9 directly to your employer with required identification.
